Creating Add button and array

hello,

     sure has been brought before still not clear on procedure still new this. in coldfusion application need create button 'add' function.

when button clicked new text field checkbox created, however, these need have unique names because submitted mysql database table. of course, unless dynamic variable can created , submitted.

 

i think need array function in javascript/ajax?

 

i appreciate help.

instead of using javascript, have add button submit form, , use coldfusion create new form elements require.
